Database schema improvements

* Do not create columns as AUTOINCREMENT (doesn't work well with
    INSERT OR IGNORE).  We still need to fix existing databases.

* Allow automatically adding columns to schema.

* Add various columns to commods, and commodclasses table, for ordering etc.
