Perhaps not a conventional smartcard, but in principle you could have 
some sort of necessary token stored on a micro-HD card plugged into an 
Android device. Such a scheme might or might not be proof against an 
attacker gaining a simple copy of that data (and the Android system 
allows remote interrogation of the micro-HD card without very much in 
the way of credentials).

A better idea might be some sort of "Cryptocard" token, are they 
approved for NHS use?
