While fashionable to blame the police, the blame seems to rest
squarely with the IPCC for this[1]:

    Mark Duggan death: IPCC 'may have misled journalists'

    The police watchdog said there was no evidence Mr Duggan had fired at police
    The police watchdog has admitted it may have misled journalists
into believing police shooting victim Mark Duggan fired at officers
before he was killed.
