I wouldn't say the pattern is atypical.  It occurs when the fraudster
is close to the cardholder (spouse, relative, healthcare worker) and
has gained access to the card and the PIN.  In general, it is assumed
that such access was obtained with consent from the account holder or
due to negligence, so the bank is not liable.
