securing distributed partial medical records?

Suposing we don't hoover(TM) everyone's entire medical record up from
the several places that pieces of it are at present into one large new
system for the whole world (currently for world read "England").
Supposing we set up certain services where someone goes, and has a photo
of the back of their eye taken whcih is of interest to their opttican
and ophthalmologist, to their GP, to their endocrinologist, and to any
physician wondering if their hypertension is significant; or of a skin
lesion using clever imaging that looks below the surface a little; or an
ECG or a chest xray.
I like the idea of leaving those images, that data, the vector of the
ECG and the conclusions of the artificial and real intelligences that
look at it where they are, but passing a pointer to other systems that
need, want opr should assemble those pieces togehter to show to a doctor
looking after teh whole patient.
How do we secure it adequately?
