CAP consultation and extension of access to PDS

On a personal note, how do I verify what data is held about me in the
PDS? Is this achieveable with a normal DPA request?
Of particular interest is the "NHS Care Record consent to share
status" flag as I do not recall ever being asked that question.

Presumably it defaults to "yes"?
