Their current T&Cs are long and dreary and linked here:

do?categoryId=3DCON-TOTAL-BB-R1&s_cid=3Dcon_FURL_broadband
Section 9 seems most appropriate:
Ending the service
=95 When you end your broadband service outside your minimum =
period and =20
do not request and use a migration access code to move to another =20
service provider, you will have to pay a cease charge of =A36. You will =20=
not have to pay this charge in the event you are moving home and we =20
are unable to provide the service at the new address.
=95 Unless we have made a change to the prices or terms and =
conditions =20
that is to your material disadvantage or paragraphs 9 and 10 of the BT =20=
Total Broadband service terms apply you must pay a charge for ending =20
BT Total Broadband within the minimum period by way of compensation to =20=
us. This charge will be equal to the total of the monthly rental =20
charges left in the minimum period. This charge will not be subject to =20=
VAT.
=95 We may also charge you up to =A345 for the home hub and =A3150=
for the =20
mobile phone(s) we have provided to you as part of the service.
Bullet point 2 - 'Unless we have made a change to the prices or terms =20=
and conditions that is to your material disadvantage' might just be =20
considered to cover Phorm.
