pedantic; the salaries of the anesthetist, surgeon, nurses, patient  
care, etc are all billed up into the price of a procedure, since this  
is the most efficient way of dealing with medical practice billing and  
is used in more than one market. Still, given the way the prescription  
authority works, the lack of central purchasing for drugs and the  
whole existence of NICE for drug rationing, i think you should look  
again.
