Sorry, I thought I'd got it - confusion from my downloaded version. It's
part of a set of three documents:

and the original
"Managing Virtual Memory in Win32", which now seems to have disappeared,
although I'm happy to find this for you if you'd
like to see it (it was on the site recently - copyright shows 2006 on my
local copy)
These used to be given as the standard texts for Windows memory management
(by Randy Kath).
