A lot of the problems of Climbie were down to individuals filing
information away and not doing anything to substantiate it or act upon
it.  I also would dispute if doctors should be receiving third party
information, holding it or acting upon it anyway: their intervention
would greatly complicate using the stuff evidentially.  Why wouldn't a
doctor is receipt of such information pass it immediately to social
services?  After all, the patient-doctor confidentiality issue isn't an
issue, as this information didn't come from the patient.

By the way, I presume you mean ``Climbie-type''.  Holding Climbie up as
an example of good practice would be, well, an interesting
interpretation.  Yes, I have read the report.  Every page of it.
