It wasn't a Y2K bug (because it was approx 1996) but I've encountered a 
hospital outpatients department that wouldn't make any bookings because 
of a glitch with their booking system where the fault meant that sums 
like <today>+<90 days> routinely gave a clearly preposterous answer (and 
for some reason that kind of formula was the only way they could book 
future appointments).
