I have seen this too - but only for the Internet Explorer plugin (and as far
as I can tell, the fault is on ie's side - no guarantee there though)
if you use bhocop (or similar) to remove the plugin from ie, then you are
presented with the classic "save or open" dialog; this works reliably 100%
of the time no matter which option you chose (given it will download then
open as a standalone app)
I have never seen any problems at all with moz - but then, I haven't noticed
there being a "embedded" plugin for it either (I may have missed it of
course :)
