They will expect whoever issued the card to keep a backup. Which begs 
the question of who will be authorised to update the card, and whether 
they also keep an archive dopy of the update, and whether that's kept 
locally or centrally.

The other thing people are missing is that Doctors also look at people's 
medical records when the patient isn't present.
