I don't see what you need the smart card for.
Carrying your own paper records would offer some benefits.
For example:
I can carry a photocopy with me on holiday *and* simultaneously maintain a
backup copy at home.
CD-R seems to also be a better candidate.

The last 4 GPs that I have visited, in England, Scotland and California,
have not had my medical records. I don't know about the one before that.
So I support this idea.
Just not with smartcards.
