My understanding is basically:

Databases owned/operated by Transport for London.  At the end of the day
reconciliation is made between the car registrations recorded and the registration numbers of vehicles that have paid for that day.

Various complications are raised by classes of vehicles such as NHS ambulances,
other emergency vehicles, vehicles used for private or organisational purposes (cars used by doctors, social workers, etc..), and per use charging of fleet vehicles (corporate or hire company).

Billing for vehicles without matching payment is sent c/o DVLA.
