I'm only commenting on the debian changelog here, my reply should not
keep others from commenting too.

Apparently you copied the upstream changelog entries to
debian/changelog. The upstream changelog will be available at
e.g. /usr/share/doc/sysvinit-core/changelog.gz though, so no need to
duplicate these.

It suffices to list changes made by the packagers. Some say that only
listing user visible changes is enough, while some also include
important upstream changes (see the debian-changelog of linux-image-*
for an example).
